When combining multiple pdf's into a single document, is there a way to output "fit two pages" but have page one display alone? 

I would like to do this so that the resulting document (when viewed onscreen) looks like a printed document, with even number pages on the left and odd number pages on the right.  This keeps the proper layouts all thru the document.

Right now, if I specify "fit two pages" and "startup page=1", the resulting document puts page 1 on the left and page 2 on the right.  All of the rest of the pages are shifted accordingly and completely screw up the layouts.

My work-around has been to add a blank page at the start of the document but I would like to do away with this extra and unprofessional looking setup.

Thanks for this great program.  It is much more compact, easier to use, and generates much smaller file sizes than others I have tried.
